James Reece passed away at his home in Accrington 💔

A mum has voiced her concerns after pages were ripped from a notebook found after her son took his own life.

The inquest heard extracts from a statement by Deborah in which she described her son's long-term mental health issues. James, who was born in Blackburn, suffered from attention deficit hyperactivity disorder and doctors also suspected he had autism and Asperger Syndrome. Family friend Holly Jenkins, who was also with James the night before he died, said he had told her that he doesn't "want to live this life anymore" and that he needed "a gun so it's painless". Despite this Holly said: "I never thought for a second that he was going to kill himself that night."

James' mum said she had fought for five years to secure treatment ADHD treatment since he had turned 18. The transition "never happened" and James was only prescribed medication levels suitable for a child. Deborah described Haydock Medical Centre, the Merseyside-based surgery outsourced to provide ADHD assessments in Lancashire, as "diabolical" and accused them of "telling lies" by claiming to have attempted to contact James. She added: "I told them that they would have to ring me first, because James wouldn't answer the phone to an unknown number, and they said he hadn't answered and knocked him off the list but that's a lie.

Four days later, James was called by a mental health practitioner called Trevor from The Mount in Accrington but James refused to talk. The practitioner said that he did not consider that James was a risk to himself or others. But the inquest heard that Trevor failed to complete a risk assessment or care plan and should have considered ringing Deborah or carrying out an unannounced visit.
