An elderly woman who was killed on a rail crossing had been unable to see the train because it was hidden by another train going the other way.A report by the Rail Accident Investigation Branch said Network Rail knew the crossing, near Ashtead, posed an "unacceptable risk".The 85-year-old victim was crossing with a dog and pushing a wheeled trolley bag.
The woman responded to hearing the second train's horn by "hurrying forwards" but was "unable to get clear of the path of the train in time", the report said. The report concluded that the woman's view of the train was blocked by another train
