Wales NHS: Family's lives 'destroyed by death of UHW patient' - ombudsman
The report says the patient's clinical history and new symptoms were not adequately assessed
A report by public services ombudsman Michelle Morris said the care he received amounted to a "significant service failure".The ombudsman wrote: "I have no doubt that the findings of this report will be a source of great distress to her and her family". In the complaint, the daughter said the family's lives were destroyed by the patient's death and that they still had no answers.
Following the surgery, the patient was not moved to the Intensive Care Unit despite being very unwell.'Improvement plan'
