The two continents, America and Europe, are among the most affected. The current wave of mpox cases has infected chiefly men who have sex with men . One of the common symptoms of mpox is anogenital lesions, which indicate transmission of the virus through sexual activities.

Although mpox vaccination has been recommended at both primary vaccinations, i.e., pre-exposure to the virus, and post-exposure preventive vaccination , there exists scarce evidence that supports the effectiveness of these two prophylactic strategies. The contact-tracing strategy was used to identify at-risk contacts. Additionally, some individuals self-declared and were available for vaccination. A total of 130 adults received MVA-BN as part of PEPV between June 15 and August 12, 2022. Among these, 108 participants received a single dose of the MVA-BN vaccine within fourteen days of mpox exposure. These participants were included in the current study.

Clinicians confirmed breakthrough infection through diagnostic consultations via real-time PCR on the skin and/or mucosal swabs collected from lesions. The mpox virus was isolated from samples that tested positive for mpox infection by PCR to assess virus infectivity. Although initial Cq values of samples from non-vaccinated mpox patients and vaccinated individuals with breakthrough infection were comparable, viral culture in the latter group was observed to turn positive in due course. Therefore, more research is required to validate this finding.

Axonal degeneration occurs early, preceeding and exceeding neuronal loss, and contributes to the spread of pathology, yet is poorly described outside the nigrostriatal circuitry. The insula, a cortical brain hub, was recently discovered to be highly vulnerable to pathology and plays a role in cognitive deficits in PD and DLB. The aim of this study was to evaluate morphological features as well as burden of proteinopathy and axonal degeneration in the anterior insular sub-regions in PD, PD with dementia (PDD), and DLB. Methods α-Synuclein, phosphorylated (p-)tau, and amyloid-β pathology load were evaluated in the anterior insular (agranular and dysgranular) subregions of post-mortem human brains (n = 27). Axonal loss was evaluated using modified Bielschowsky silver staining and quantified using stereology. Cytoskeletal damage was comprehensively studied using immunofluorescent multi-labelling and 3D confocal laser-scanning microscopy. Results Compared to PD and PDD, DLB showed significantly higher α-synuclein and p-tau pathology load, argyrophilic grains, and more severe axonal loss, particularly in the anterior agranular insula. Alternatively, the dysgranular insula showed a significantly higher load of amyloid-β pathology and its axonal density correlated with cognitive performance. p-Tau contributed most to axonal loss in the DLB group, was highest in the anterior agranular insula and significantly correlated with CDR global scores for dementia. Neurofilament and myelin showed degenerative changes including swellings, demyelination, and detachment of the axon-myelin unit. Methods Ecological study in the 12 largest municipalities in Andalusia (Spain) during the first three epidemic waves of the COVID-19 (02/26/20—03/31/21), covering 2,246 census sections (unit of analysis) and 3,027,000 inhabitants. Incidence was calculated, standardised by age and sex, for infection, hospitalisation and deaths based on average gross income per household (AGI) for the census tracts in each urban area. Association studied using a Poisson Bayesian regression model with random effects for spatial smoothing. Results There were 140,743 cases of COVID-19, of which 12,585 were hospitalised and 2,255 died. 95.2% of cases were attributed to the second and third waves, which were jointly analysed. We observed a protective effect of income for infection in 3/12 cities. Almeria had the largest protective effect (smoothed relative risk (SRR) = 0.84 (0.75–0.94 CI 95%). This relationship reappeared with greater magnitude in 10/12 cities for hospitalisation, lowest risk in Algeciras SRR = 0.41 (0.29–0.56). The pattern was repeated for deaths in all urban areas and reached statistical significance in 8 cities. Lowest risk in Dos Hermanas SRR = 0.35 (0.15–0.81). Conclusions Income inequalities by geographical area were found in the incidence of COVID-19. The strengths of the association increased when analysing the severe outcomes of hospitalisations and, above all, deaths.
