As our population ages, the complexity of patient care has increased dramatically over the last 30 years. Many family physicians feel ill-equipped to deal with the diversity and intensity of the “cognitive load” of family medicine/primary care today. Family doctors manage their clinics and pay all the expenses for overhead.
The fee-for-service remuneration model for doctors is not the best system. “Capitation” brings better outcomes at lower cost than fee-for-service or salaried models. In capitation models, the family doctor is paid one annual fee for each formally attached patient and all primary care must be provided for that fee.
