The National Transportation Safety Board’s 19-page report is not intended to render judgment on who, if anyone, made mistakes that may have led to the incident on the Boeing 737-9 MAX.

The investigators said that the lack of certain damage around the panel indicates that all four bolts were missing before the plane took off from Portland, Oregon.Without the bolts, nothing prevented the panel from sliding upward and detaching from “stop pads” that secured it to the airframe. Although the preliminary report noted Spirit’s role in the incident, Boeing CEO David Calhoun said last week that the company caused the problem.
